Did you know that it is your employer/union that determines your insurance plan?

 

If you wish to have more Chinese Medicine and/or Massage Therapy coverage, this needs to be directed to the human resource department of your company or union of your work place.
 

In order to help this process, we have complied letter templates below for you/patients to send to their employer or union to request your wish.

Please edit as needed (especially the part in RED) and submit it to them.
